The next page of the
Wizard shows the Question group that we have already selected and allows us to select
additional question groups.
For our example, we
will select NEXT and move on to the next page in the Wizard.

On this page of the
Wizard, we can select formatting options. We can further alter the format once the Wizard
generates the form.

The next page of the
Wizard is perhaps the most confusing, (this is the BETA 2 version of Office 2000 and this
may be fixed in the official release).
While we want the
results of the form to be stored in a database, the option does not exist. We will select:
"USE CUSTOM CGI SCRIPT" for now and change it to a DATABASE in the next step.

form, select FINISH to have FrontPage generate the form and display it in the EDIT window.
