First we need to
create a form to allow us to input data. Select FILE from the MENU BAR, then NEW, then
PAGE.

From the NEW PAGE
window, select the "FORM PAGE WIZARD".

The first page of
the FORM PAGE WIZARD informs us that it will walk us through the process of creating a
form.
|
|
Groups of standard
questions can be inserted into our form using the next page of the Wizard.
For this example we
will select "CONTACT INFORMATION".

We have an
opportunity to DE-SELECT some of the "CONTACT INFORMATION" fields on the next
page of the Wizard.
Here we have
DE-SELECTED "Home_ Phone" and "Postal Address".
